Question
This image is from a young dog with a grade 4/6 left basilar systolic murmur. What does the image show?
A: Left ventricular concentric hypertrophy and dilated left atrium
B: Right ventricular concentric hypertrophy and right atrial dilation
C: Right ventricular concentric hypertrophy and Ebstein’s anomaly
D: Right and left ventricular concentric hypertrophy
Answer
The correct answer is B.
This image shows evidence of concentric hypertrophy which develops in the presence of increased afterload. In this case pulmonic stenosis is the cause of the increased afterload on the right ventricle.
Pulmonary hypertension may cause a similar change but is usually seen in older animals. Before diagnosing pulmonary hypertension it is important to exclude right ventricular outflow tract obstruction, of which pulmonic stenosis is one cause.
The left side can appear to be hypertrophied in cases of pulmonic stenosis and pulmonary hypertension. However it is not true hypertrophy and is termed ‘pseudohypertrophy’ as reduced right ventricular cardiac output, secondary to severe stenosis/ hypertension, results in an under-filled left ventricle. Furthermore, septal flattening is also present caused by the high right-sided pressure which makes the left side appear smaller.
